Request Amendment
If you believe your health records or personal information held by Elderwise contain errors or are incomplete, you have the right to request an amendment.
What Can Be Amended
Under HIPAA, you may request amendments to your protected health information in the following areas:
Personal Details
Corrections to your name, date of birth, contact information, or other demographic data.
Medical History
Corrections to diagnoses, conditions, allergies, or medication records that are inaccurate.
Assessment Records
Updates to geriatric assessment results, care observations, or clinical summaries that contain errors.
Care Plan Information
Amendments to care plans, treatment notes, or provider information that is incorrect or outdated.
How to Request an Amendment
Submit a Written Request
Email privacy@elderwise.ai with the subject "Amendment Request." Include a clear description of the information you believe is incorrect and the proposed correction.
Provide Supporting Documentation
If available, include any supporting documents or evidence that substantiates the requested amendment. This helps expedite the review process.
Review and Decision
We will review your request and either accept or deny the amendment. If denied, we will provide a written explanation and inform you of your right to submit a statement of disagreement.
Processing Time
Days Maximum
Amendment requests are processed within 60 days of receipt. If we need additional time, we will notify you of the delay and the reason.
You will receive an acknowledgment within 5 business days of submitting your request. If the amendment is accepted, we will make the correction and notify all parties who previously received the incorrect information.
