Request Disclosures
You have the right to receive an accounting of disclosures -- a record of when, to whom, and why your protected health information was shared by Elderwise.
What the Disclosure Log Includes
The accounting of disclosures provides transparency into how your health information has been shared. It includes:
Date of Disclosure
The specific date on which your protected health information was disclosed to a third party.
Recipient Information
The name and address of the entity or person who received your health information.
Description of Information
A brief description of the health information that was disclosed in each instance.
Purpose of Disclosure
The reason for the disclosure, including any legal basis or authorization under which it was made.
Exclusions
The accounting does not include disclosures made for treatment, payment, or healthcare operations; disclosures you authorized in writing; disclosures made directly to you; or disclosures for national security or law enforcement purposes as permitted by law.
How to Request a Disclosure Log
Submit Your Request
Email privacy@elderwise.ai with the subject "Disclosure Accounting Request." Include your full name and the time period you would like the accounting to cover (up to 6 years).
Identity Verification
We will verify your identity to ensure the request is legitimate before compiling the disclosure log.
Receive Your Log
The disclosure accounting will be provided in your preferred format (electronic or paper). The first request in a 12-month period is free; subsequent requests may incur a reasonable fee.
Processing Time
Days Maximum
We will provide the accounting of disclosures within 60 days of your request.
If we need additional time, we will notify you in writing of the reason for the delay and the date by which the accounting will be provided. Only one extension of up to 30 additional days is permitted.
