What Restrictions Mean
A restriction limits how Elderwise uses or shares your protected health information. You may request restrictions on:
Treatment Uses
Limit how your information is used in providing care recommendations or shared with healthcare providers.
Payment Disclosures
Restrict sharing of your health information for payment or billing purposes with third parties.
Healthcare Operations
Limit the use of your data in quality improvement, analytics, or operational activities.
Specific Disclosures
Prevent disclosure of specific information to certain individuals, such as family members or other caregivers.
How to Request a Restriction
Submit a Written Request
Email privacy@elderwise.ai with the subject "Restriction Request." Clearly describe the specific restriction you are requesting, including what information it applies to and what uses or disclosures you want limited.
Review Process
We will review your request and evaluate whether the restriction can be accommodated while still providing effective care and meeting our legal obligations.
Confirmation
If we agree to the restriction, we will document it and ensure all relevant staff and systems comply. You will receive written confirmation.
Our Obligations
While Elderwise is not required to agree to all restriction requests, there are important exceptions:
- -- We must agree to restrict disclosures to a health plan for services you paid for entirely out of pocket.
- -- If we agree to a restriction, we are bound by it unless you revoke it in writing or in emergency situations where the information is needed to provide treatment.
- -- If we cannot accommodate a restriction, we will explain the reason and discuss alternative options to address your concerns.
