You have the right to access and obtain a copy of your medical records and personal health information maintained by Elderwise. Here is how to submit your request.
What Records Are Available
You may request access to the following categories of health information maintained in your Elderwise account:
Care Observations
Daily care logs, caregiver notes, and behavioral observations recorded through the Elderwise platform.
Assessment Results
Geriatric assessment scores, cognitive screening results, and functional evaluations.
Clinical Summaries
AI-generated clinical summaries, health trend reports, and care recommendations shared with your clinicians.
Account Information
Your profile data, communication preferences, consent records, and any correspondence with our support team.
How to Request Your Records
Submit a Written Request
Email privacy@elderwise.ai with the subject "Records Access Request." Specify which records you would like to access and your preferred format (electronic or paper).
Identity Verification
We will verify your identity to protect your information. You may be asked to provide details associated with your account.
Receive Your Records
Records will be provided in your requested format. A reasonable, cost-based fee may apply for paper copies.
Processing Time
Days to Complete
Your records request will be processed within 30 calendar days of identity verification.
You will receive an acknowledgment within 5 business days. If we need additional time (up to 30 more days), we will notify you in writing of the reason for the delay. Electronic records are typically available faster than paper copies.
